Extraction of Mesiodens.Case report by Dr Azhar Chisti

27 year old patient,Shashi came to me with an unusual complaint of sharp tooth cutting his tongue.On examination a supenumerary tooth was found with a sharp incisal edge and tilted palatally.The supernumerary tooth which is usually named as a mesiodens(that supernumerary tooth which is present between the two upper central incisors) in dental terminologies had a sharp edge causing an ulcer to the tongue.Radiograph showed the root of mesiodens entangled between the roots of two central incisors which can cause a big problem during the extraction of this tooth.
Hence it was planned that the tooth be extracted without making any mesio-distal or bucco-palatal movement as it may cause unwanted movement of root tips of incisors.The tooth was extracted after proper seperation of PDL fibres with pure rotational movements
